the way i've always seen it is as a combination spell/tattoo. i've read a handful of stories that include wizarding tats - firequill tattooing, moving tattoos, w/e - and i imagine in like that. perhaps only other marked DEs can see it? (have we ever actually seen one in a book?) so when it's tattooed in your skin - maybe a potioned ink or something - it "recognizes" the other DEs. clearly, Voldemort uses it to notify DEs, so it has to be magical.

We saw Pettigrew's, too. In the graveyard scene, Voldemort uses it to summon the other DEs.
